Residency Requirement
Patients must be residents of Texas or New York to receive care at this clinic.
Initial Evaluations
Controlled Substance Prescriptions
- If you require a controlled substance prescription, your initial evaluation must be conducted via Telehealth, with monthly Telehealth follow-ups.
Stimulant Prescriptions
- We will first explore non-stimulant treatment options before considering a stimulant prescription.
- If a stimulant is recommended, you will be required to complete an EKG and a drug screening before a prescription can be issued.
- Stimulant prescriptions are at the provider’s discretion and based on a full assessment.
- The provider reserves the right to deny any medication if deemed inappropriate for your condition.
Non-Controlled Prescriptions
- If you do not need a controlled substance, your visit will be conducted via Telehealth.
Change in Prescription Needs After a Virtual Evaluation
- If your initial virtual evaluation determines that you require a controlled substance, your next follow-up appointment will also be via Telehealth within 14-30 days, depending on the provider’s assessment.
- Controlled substance prescriptions are subject to both the provider’s and pharmacy’s discretion, and a Telehealth visit may be required before the prescription is filled.
Follow-Up Appointments
- All follow-up appointments will be conducted via Telehealth unless a change in treatment requires additional evaluation.
Cancellations & Rescheduling
- Please notify us at least 48 hours in advance if you need to cancel or reschedule your appointment. No cancellation fee will be charged for timely notices.
- If you no-show or cancel on the same day, a $100 fee will be charged for the missed appointment.
FMLA and Other Documentation Requests
FMLA (Family and Medical Leave Act) Documentation
Please note that FMLA documentation will only be completed for patients after they have had a minimum of three visits and are established with our practice. There is a $199 administrative fee for the completion of FMLA paperwork. This fee is not billable to insurance and must be paid in full prior to the release of any documents.
ESA Letters, Work Accommodations, and Testing Accommodation
Requests for Emotional Support Animal (ESA) letters, work accommodations, and testing accommodations are available upon request for a $99 administrative fee. Patients must be established with our practice and must have attended at least two appointments prior to any documentation being completed.
Turnaround Time
Please allow 5–7 business days for the completion of all documentation requests.
